"Efforts by the passenger to move from the track bed to the under-platform recess were blocked by communication cables. CCTV showed that the passenger, who was described as vulnerable, was aware of the approaching train before they lost balance and fell onto the track. They were undiscovered for two minutes and not spotted by station staff nor the train driver prior to being hit."

The passenger remained undiscovered for two minutes, undetected by station staff and the train driver due to poor lighting conditions and dark clothing. The Rail Accident Investigation Branch identified three key recommendations: updating safety standards for under-platform recesses, completing platform-specific risk assessments, and implementing measures to reduce platform-fall risks. Concerns were also raised about delays in post-incident drug and alcohol testing procedures.
