"Late Limp Bizkit bassist Sam Rivers was found dead in a blood-stained bathroom earlier this month, according to a new report Thursday. Per a recording of the 911 call obtained by TMZ, a woman identified only as Keely told a dispatcher she discovered Rivers face first in a pile of blood in a bathroom at his Florida home. She can be heard attempting to perform CPR during the call as she sobs into the phone."

"Responding officers from the St. Johns County Sheriff's Department also performed CPR before paramedics arrived and pronounced Rivers dead. The 48-year-old musician appeared to have a cut above his eye, suggesting he may have suffered a fall, according to an incident report also obtained by TMZ. A deputy noted Rivers' face and neck were blue common symptoms of a pulmonary embolism, a blood clot that blocks and stops blood flow to the lungs. However, his cause of death has still not been determined."
