Prescribed melatonin use was associated with higher likelihood of hospitalization for heart failure and greater risk of dying from any cause. Heart failure occurs when the heart cannot pump effectively, which can cause arrhythmias, liver damage, and death, and is managed with medications and lifestyle changes. Melatonin is an endogenous hormone that regulates the sleep–wake cycle and is available over the counter and by prescription. The analysis showed consistent increases in serious outcomes after adjusting for many risk factors. Key limitations include reliance on electronic medical records, untracked over‑the‑counter use, unknown participant doses, and that association does not prove causation.
