Testosterone replacement is available in the United States for people with low hormone levels caused by known medical issues. Evidence suggests additional people may benefit from testosterone delivered through injections, patches, subcutaneous implants, or gels. An FDA expert panel recommended major policy changes to expand access for people with a range of conditions and described testosterone replacement as preventive health and a multibillion-dollar preventive-care opportunity. Clinicians disagree on eligibility. Some argue that most men with low testosterone, especially young men without a medical cause, should improve levels through lifestyle changes and weight loss. Others say symptomatic low testosterone, including low libido, fatigue, and irritability, may warrant therapy. More enthusiastic proponents recommend testing all cis men and treating those with low levels even without symptoms.
