"Her mother has criticised hospital staff for their "indifference, ignorance, even at times cruelty"."
"Her death at such a young age in the circumstances in which it occurred is a terrible tragedy."
"I am in no doubt that there was a complete failure to adequately assess and manage the risk that this posed."
"Keeping the communal area temporarily locked while Alice was on the ward would not have posed a problem beyond one of "inconvenience"."
Alice Figueiredo, 22, died by suicide in July 2015 while detained on a mental health ward at Goodmayes Hospital after more than ten prior attempts. Staff failed to remove plastic items from communal toilets despite her known self-harming, creating an accessible means that contributed to her death. North East London NHS Foundation Trust was convicted of failing to ensure the safety of a non-employee and fined £565,000 with £200,000 costs. Ward manager Benjamin Aninakwa was convicted of failing to take reasonable care, given a six-month prison sentence suspended for 12 months and 300 hours unpaid work.
