Soleiman Faqiri, 30, who had schizoaffective disorder, was taken into custody on Dec. 4, 2016 after allegedly stabbing a neighbour during a psychotic episode. Less than two weeks later he died while awaiting a mental health assessment at Central East Correctional Centre near Lindsay, Ont. He was found face down on a cell floor after guards punched and struck him repeatedly, pepper sprayed him twice, covered him with a spit hood and left him shackled. A coroner's inquest two years ago ruled the death a homicide and issued 57 recommendations. Family members report no apology or engagement from the province and say recommendations remain unimplemented.
